Personalized Health Plan for the prevention and self-management of asthmatic attack.

Relevant Information: Asthma is an incurable medical condition that waxes and wanes, with periods in which the patient is almost completely free of symptoms, and periods when the patient experiences multiple severe attacks, breathes with wheezy sound, and continuously finding it difficult to breath. The escalation of the symptoms of asthma happens when the patient is exposed to certain irritants, which are not the same for all asthmatic patients, but include exercise, change in weather, mold and strong smells such as those of perfume and onion. All these are taken into consideration in the formulation of a Personalized Health Plan for an asthmatic patient. Personalized Health Plan takes into consideration the triggers of the patient’s asthmatic attack, and then make arrangement on how the patient can avoid or get used to the triggers. The plan also includes an emergency plan on how to deal with a severe asthmatic attack that is non-responsive to the usual treatment.

Annual comprehensive medical examination for asthmatic patients

Relevant Information: Asthmatic patients might be without symptoms for many months, only to suddenly have a severe asthmatic attack that threatens his/her life. There is therefore a need to predict the likelihood of the severe asthmatic attack, and to set up a mechanism that not only prevent the severe attack, but is also able to treat the severe attack, to save the life of the patient. It is recommended that asthmatic patients should have at least an annual comprehensive medical examination to assess the state of the patient’s lungs, evaluate the effectiveness of their treatment plan, and if necessary, to overhaul their personalized health plan.

Self-Management Education programme

Making your Personalized Asthma control plan work for you

Relevant Information: Asthmatic attacks are often sudden and can occur in locations and at times that make it difficult to access health care in a hospital. There is therefore a big need for the patient and his/her caregivers not only to know how to prevent the severe asthmatic attacks, but also how to administer the needed treatments, to cure the attack. Patients and their caregivers need to be trained on how to identify the early symptoms of a severe asthmatic attack; taught how to use the home peak flow meter, to assess the state of their lungs and predict when an attack is imminent; and also need to trained on how to use the inhaler and the other gadgets in the treatment of asthmatic attack.

Peak Flowmeter

Relevant Information: A peak flow meter is a portable, easy-to-use device that measures how well the lungs are able to expel air. By blowing hard through a mouthpiece on one end, the peak flow meter can measure the force of air in liters per minute and gives a reading on a built-in numbered scale. Peak flowmeter is recommended for asthmatic patients to enable them keep track on their asthma control, by detecting airway narrowing even before they feel any symptoms, giving them time to adjust their medication or take other steps before the symptoms get worse.

Nebulizer

Relevant Information: A nebulizer changes medication from a liquid to a mist so that it can be more easily inhaled into the lungs. Nebulizers are particularly effective in delivering asthma medications to infants and small children and to anyone who has difficulty using an asthma inhaler. It is also convenient when a large dose of an inhaled medication is needed. Nebulized therapy is often called a “breathing treatment.” And a variety of medications — both for immediate relief and maintenance of asthma symptoms — are available for use with a nebulizer. Nebulizers come in home (tabletop) and portable models. Home nebulizers are larger and must be plugged into an electrical outlet. Portable nebulizers run on batteries — either disposable or rechargeable — or can be plugged into a car’s cigarette lighter. Smaller, portable units are slightly larger than a deck of cards, so they can be carried in a purse, briefcase, or backpack to be used whenever and wherever you need them.
