Personalized Health Plan for the management of hypertension

Relevant Information: Hypertension often does not have any symptom, and gives the patient an impression that all is well, only to manifest with deadly consequences as stroke, heart attack, heart failure among others. It is therefore difficult to treat and control, because the patient feels no symptom, and therefore have little or no motivation to seek treatment, and to comply with treatments that often have uncomfortable side effects. All these are taken into consideration in the formulation of a Personalized Health Plan for a patient with hypertension. Personalized Health Plan is a detailed plan of what the patient has to do, and what has to be done for the patient, to ensure a long and healthy life, in spite of the disease. It is an international best practice, not too common in Nigeria, but highly recommended for every patient with hypertension, as soon as they are diagnosed.

Periodic Medical Check-up of patient with hypertension

Relevant Information: Hypertensive patients rarely go to the hospital. They have no symptoms, and they use diet, physical activity and drugs to control their blood pressure, which they can administer to themselves, without the assistance of a health worker. Hypertensive patients are however expected to visit their Personal Physician every three months, for medical check-up, if they have any symptom suggestive of any of the complications of hypertension. Hypertensive patients are expectedto only have acomprehensive medical checkup once every year, if they are without any symptom of the disease.

Both the quarterly and the comprehensive medical check-ups are carried out by with the input of a Cardiology, and are designed to identify the complications, and then package a personalized health plan to prevent the escalation of the identified complication.

Use of diet in the self-management of hypertension

Relevant Information: Diet is the best “drug” for the treatment of hypertension. This is because it has no side effects, unlike the anti-hypertensive drugs that make the patient urinate often, drowsy and sometimes even causes impotency, even to the most virile man. The diet that has been found to be effective against hypertension is called the DASH diet, DASH being an acronym for Dietary Approach to Stop Hypertension. DASH diet is low in salt, which means that the diet is likely to be tasteless and unpalatable. It also contains lots of fruits and vegetables, for their potassium, calcium and magnesium contents, which makes it different from the Nigerian staple foods. Patients with hypertension and their caregivers therefore need to be trained to use salt substitutes in their cooking, and taught how to prepare palatable meals from the local staple food crops, without going against the recommendations of the DASH diet. These would be taught in this training module.

It is a one-day programme, delivered by experienced health professionals using such learning methods as didactic lectures, video and practical demonstrations.The training is held in hospitals, churches, mosques, gyms, public buildings, and other venues that are very convenient for diabetic patients, to ensure that all diabetic patients and their caregivers are able to receive this very important training.
